Gagal jantung merupakan penyakit kronis yang menjadi masalah utama di dunia. Pasien dengan gagal jantung seringkali harus dirawat inap berulang karena perburukan atau kekambuhan gejala gagat jantung seperti nyeri dada, sesak nafas dan edema akibat keleblhan volume cairan. Rawat inap berulang menjadi prediktor morbiditas dan mortatitas gagal jantung. Kejadian rawat inap berulang dapat dicegah dengan adanya perawatan yang bersifat multidisiplin dan terintegrasi datam hal monitoring, manajemen mandiri di rumah, manajemen gejala, dan kegawatdaruratan. Buku ini ditulis oteh professional kesehatan multidisiplin yang memberikan wawasan mengenai pengembangan perawatan mandiri dan multidisiplin pada gagal jantung. Buku ini diawali dengan penjelasan mengenai dasar-dasar gagal jantung, strategi perawatan mandiri oleh pasien dan keluarga dengan pendekatan multidisiplin.
