While a surgeon many decades agotreated diseases from headtotoe, this concept has evolved, and today some degree of specialization is the rule worldwide. In many countries various boards for sub-specializations are designed, and after a broad training in general surgery, many young surgeons move on further into a specific field. Knowledge of anatomy and precise surgical technique remain the foundation of high quality surgery. A knowledgeable surgeon, equipped with excellent theore- cal and clinical skills, will only be accomplished when he or she masters the - erative techniques of the practice of surgery. The legacy of an academic surgeon or a surgical teacher relies in great part onthe tra...
The first edition of this book was judged by many to be the best available atlas of upper gastrointestinal and hepato-pancreato-biliary surgery. The second edition builds upon this success by covering all of the significant intervening developments, including both new procedures and adaptations of established techniques. The atlas opens with an introductory section on the basic principles of operative surgery. All of the important surgical techniques are then presented in a series of sections devoted to the esophagus, stomach, and duodenum, the liver, the biliary system, portal hypertension, the pancreas, and the spleen. General, oncologic, and transplantation surgical procedures are described step by step with the aid of more than 900 superb illustrations drawn by a team of artists with extensive experience. The contributors were selected on the basis of their extensive experience in the procedures discussed and most are established educators.
Among malignant tumors, adenocarcinomas of the esophagogastric junction show the highest increase in incidence over the past three decades in Western industrialized countries. This special volume, with contributions from experts in the field, covers all aspects of the disease. Etiology, pathogenesis, classification, and clinical staging are discussed, and there is special emphasis on state of the art treatment techniques. The latter range from endoscopic mucosal resections or limited surgical resections for early cancers to multimodality treatment options for locally advanced tumors. Emerging quality issues in surgical management are addressed. Detailed attention is also paid to other important recent developments, including molecular response prediction, early metabolic response evaluation by PET and PET-CT, the diagnosis of micrometastases, and the use of sentinel node technology. This volume will be of interest to all clinicians concerned with the diagnosis and management of this malignancy.
Clem Imrie and Colin Johnson This book stems from an unusual meeting held in The Royal College of Physicians and Surgeons of Glasgow in March 2007. Enthusiastic doctors in the diagnosis and treatment of pancreatic diseases were arranged in small groups to discuss specific unsolved/partially clinical problems and suggest the way forward. In some instances, the recommendations were initial improved longitudinal studies, while in others better double-blind randomized st- ies. The recommendations were then presented on the second day before the total 40 plus participants who added their input. Finally, the mechanics of initiating the proposals were arranged. The editors are most thankful to the ...
