Nós, enfermeiras(os), entregamos para pacientes, colegas de trabalho e gestores dos serviços de saúde o planejamento, a implementação e a avaliação do cuidado. Em CPs, desenvolvemos um cuidado que abrange as dimensões física, emocional, espiritual e social dos pacientes e famílias que vivenciam doenças ameaçadoras de vida (em suas diferentes fases). Quando reunimos enfermeiras(os) experts em CPs para compartilhar experiências entre colegas de profissão, concretizamos o sonho de, através da educação em saúde, fomentar cuidados de enfermagem contextualizados econômica e politicamente, mas que contemplem as pessoas e suas experiências singulares nas vivências de doenças avançadas. O cenário é desafiador e caracterizado por elevada incidência de doenças complexas, de alta mortalidade e aparecimento de incapacidades que geram sofrimento multidimensional. Entretanto, as evidências científicas atuais nos trazem a certeza de que os CPs executados com maestria podem transformar positivamente as realidades de quem provê recursos, de quem oferece cuidados e, principalmente, de quem os recebe.
