Innovative Strategies and New Insights for the Treatment of Stage III Non-Small Cell Lung Cancer

Stage III NSCLC comprises a heterogeneous group of patients. It includes both resectable disease, for which the standard of care is surgery plus (neo) adjuvant chemotherapy/immunotherapy, and unresectable disease, for which the standard of care is concurrent chemoradiotherapy (CCRT) followed by one year of adjuvant durvalumab. Survival in stage III NSCLC is still poor, despite all the progress made in recent years, with the advent of immunotherapy. Many questions and unmet clinical needs are still to be addressed in both resectable and unresectable disease, in order to further improve patients' survival and limit toxicities. It is unlikely that a “one fits all” model or simply adding new drugs, represent the best solutions. Predictive biomarkers are strongly needed and identifying the patients who benefit the most from a specific treatment would be key. Adaptive strategies to spare unnecessary treatment (e.g. adjuvant therapy for patients cured with surgery alone) or to timely escalate therapy might also be important.
