A decade or so ago, many clinicians were described as having an unnecessarily 'nihilistic' view of COPD. This has certainly changed over the years... This open access book on COPD provides a platform for scientists and clinicians from around the world to present their knowledge of the disease and up-to-date scientific findings, and avails the reader to a multitude of topics: from recent discoveries in the basic sciences to state-of-the-art interventions on COPD. Management of patients with COPD challenges the whole gamut of Respiratory Medicine - necessarily pushing frontiers in pulmonary function (and exercise) testing, radiologic imaging, pharmaceuticals, chest physiotherapy, intensive care with respiratory therapy, bronchology and thoracic surgery. In addition, multi-disciplinary inputs from other specialty fields such as cardiology, neuro-psychiatry, geriatric medicine and palliative care are often necessary for the comprehensive management of COPD. The recent progress and a multi-disciplinary approach in dealing with COPD certainly bode well for the future. Nonetheless, the final goal and ultimate outcome is in improving the health status and survival of patients with COPD.

Since the 1990s, the economic development of Central and Eastern Europe has maintained high economic growth rates, seemingly leading to an era of prosperity. This very positive vision of future economic success, linked to current political backlash and a long history of economic adversity, is a thin veil of the economic “way west” for so-called transition countries. The Middle-Income Trap in Central and Eastern Europe examines the reality of the diminishing marginal utility of further international investments alongside the pitfalls of higher government spending to cultivate innovation which ultimately makes foreign capital less attractive. In this volume authors from diverse disciplinary perspectives reflect on current debates surrounding the developmental bottlenecks in East-Central Europe. Exploring the EU’s Legitimacy Crisis provides a profound analysis of the causes and the consequences of the EU's growing legitimacy problem. The prior permissive consensus in the EU has been markedly declining under persistent crisis conditions. Since the onset of the eurozone crisis the EU's governance has been narrowly driven by the semi-hegemonial leadership of Germany – manifesting itself in functionalist and technocratic policy reforms concentrated on strengthening economic governance coordination. Other crucial policy areas have been neglected as member states show decreasing solidarity and a growing emphasis on national interests in response to mounting external challenges.
