Human error is implicated in nearly all aviation accidents, yet most investigation and prevention programs are not designed around any theoretical framework of human error. Appropriate for all levels of expertise, the book provides the knowledge and tools required to conduct a human error analysis of accidents, regardless of operational setting (i.e. military, commercial, or general aviation). The book contains a complete description of the Human Factors Analysis and Classification System (HFACS), which incorporates James Reason's model of latent and active failures as a foundation. Widely disseminated among military and civilian organizations, HFACS encompasses all aspects of human error, i...

A poetic journey unlike any ever attempted before... from dismemberment by God in a dark suit, marrow sucking Hell hounds, giant purple splenic feeders on miscreants and hyperthyroidism of guillotine blades to social sexist issues of forked (tongue) penises, ovaries as consumables, and a placenta pick-up scene in a bar to humans as everyman hardware and the highest proclaimed scientists as the bulging dams of over-tight anal sphincters to the calloused feet of the lone sojourner, chains of elbows, skinning alive by chemical angels, Jungian alchemical revascularizations, and the exploding of closed minds.... All profits from the sale of this book are donated to the Muscular Disease Association (MDA).

The Just Culture Principles in Aviation Law

The Just Culture Principles in Aviation Law

  • Categories: Law

This book reviews and critically analyzes the current legal framework with regard to a more just culture for the aviation sector. This new culture is intended to protect front-line operators, in particular controllers and pilots, from legal action (except in the case of willful misconduct or gross negligence) by creating suitable laws, regulations and standards. In this regard, it is essential to have an environment in which all incidents are reported, moving away from fears of criminalization. The approach taken until now has been to seek out human errors and identify the individuals responsible. This punitive approach does not solve the problem because frequently the system itself is (also) at fault. Introducing the framework of a just culture could ensure balanced accountability for both individuals and complex organizations responsible for improving safety. Both aviation safety and justice administration would benefit from this carefully established equilibrium.

In Search of Mrs. Pearson is the story of a young physician-scientist, Jeff Pearson, who has spent most of his young life in rigorous education and training. In San Francisco in the late 1980s and early 1990s, he was exposed to the horrible realities of the AIDS plague, and his work became increasingly focused upon HIV-related neurological complications. It was also during this time that he met and married Kim. However, all of his work was leading him back to Houston, where he would be able to conduct a large clinical trial with a novel drug that could offer some hope for the deteriorating mental, motor, and other neuropsychological consequences so common in HIV infection and AIDS. Along wit...
