A systematic review (a comprehensive synthesis of the literature) was carried out using rigorous scientific methodology. The search strategies were cross-disciplinary and included international published research literature. The majority of papers included in the current review were undertaken in hospital or dental school clinics, whereas the majority of patients, certainly in the United Kingdom, are treated in a primary dental care environment.
