SONAR (Swiss Sonography Group in Arthritis and Rheuma- tism) is a musculoskeletal ultrasound expert group founded in 2008. The group has developed a semi-quantitative score for Rheumatoid Arthritis using modified OMERACT criteria for synovitis, tenosynovitis and erosions. The score in- cludes B mode and Powerdoppler mode in finger joints, wrists, elbows and knees, an erosion and tenosynovitis score and ad- ditional cartilage measurement in selected joints. 2015 we developed and introduced a semi-quantitative score for hip involvement in Spondyloarthritis and started teaching the score to rheumatologists nationwide (Sonar-Hip or CoxSo- nar Score). Many rheumatic diseases cause changes that are visible in the hands. This book provides an overview of the most common rheumatic dis- eases with a focus on their hand manifestations and includes the following topics: Epidemiology, Etiology and Pathogenesis; Hand Manifestations; Further Clinical Mani- festations; and the Diagnosis and Therapy of Each Disease. The rheumatic diseases addressed are oste- oarthritis, rheumatoid arthritis, gout, calcium pyrophosphate dihydrate deposition disease, psoriatic arthritis, reactive arthritis, systemic sclerosis and dermatomyositis/polymyositis. In the diagnosis and evaluation of musculoskeletal (msk) diseases such inflammatory and non-inflammatory joint diseases, high resolution musculoskeletal ultrasound (hrMSUS or MSUS) is a superb, precise, and validated method. Many soft-tissue structures can be seen using high resolution musculoskeletal ultrasonography, and depending on the tissue under investigation, msus can also identify a variety of pathologic alterations employing mostly linear scan probes with frequencies ranging from 5 MHz to 24 MHz (up to 70 MHz when examining entheses, nails or the skin). Targeted therapeutic injections of joints, periarticular structures, tendon sheaths or bursae are among the most important treatments for inflammatory rheumatic or inflammatory activated degenerative diseases of the musculoskeletal system. Furthermore, injections of periarticular structures are also performed in post-traumatic situations or in overuse syndromes. In this publication, we discuss indications, contraindications, the clinical and ultrasound-guided techniques preferred by the authors, and possible adverse drug reactions and side effects of intraarticular and periarticular injection.
