Surgical Treatment of Colorectal Liver Metastases

Surgical Treatment of Colorectal Liver Metastases

Surgery is the gold standard treatment of colorectal liver metastases. Patients management had a dramatic evolution during the past years: more accurate diagnostic tools and more effective chemotherapy regimens have been introduced and surgical indications have been widely broadened. A multidisciplinary treatment is now mandatory, involving oncologists, radiologists an gastroenterologists. However, many topics are still debated. In the present book all aspects of surgical treatment of colorectal liver metastases are analyzed based on a systematic, updated analysis of the literature. Aim of this book is to provide surgeons, oncologists and radiologists a clear overview of the state-of-the-art of treatment of colorectal liver metastases.

Minimally Invasive Surgery of the Liver

This book intends to provide a comprehensive and up-to-date description of the newest techniques in minimally invasive liver surgery. The treatment of liver diseases comprises a significant component of the practice of any general surgeon working either in the academic, general and community hospitals. With the rapid expansion of knowledge and technology also the liver has been approached laparoscopically in a few specialized centers with well recognized advantages for the patients. Now this large amount of experience will be made universally available through this book. After a concise description of the main tools and technology necessary to carry out a safe laparoscopic and/or robot-assis...

Diagnosis of benign liver tumors is experiencing exponential growth, mainly owing to the diffusion of more accurate imaging techniques. This monograph examines the epidemiology, histopathology and genetics of these tumors and provides a systematic overview of their evolution and differential diagnosis. Care is taken to document surgical indications accurately, with a view to assisting in the avoidance of useless interventions. Open, laparoscopic and robotic surgical techniques are described and illustrated, emphasizing the optimization of intra- and postoperative management in order to avoid potential complications and degeneration. In these chapters, attention is drawn to the ways in which the surgical management of benign liver tumors differs from that of malignant tumors. The book concludes by examining the role of interventional radiology in patients with benign liver tumors.

This book describes the current applications of the robotic system in general surgery, focusing on the technical aspects of the procedures most commonly performed by this means. It provides a comprehensive overview of the state of the art in robotic surgery and presents the most recently available evidence as documented in the literature. The opening chapters review robotic platforms, discuss the general advantages, limitations, and strategies of robotic surgery, and identify challenges and critical elements when setting up a robotic program. The full range of applications of the robotic system is then covered, encompassing thoracic, esophageal, gastric, hepatobiliopancreatic, splenic, color...

Ultrasound-Guided Liver Surgery

Ultrasound-Guided Liver Surgery

Ultrasound guidance of liver surgery is a very sophisticated approach that permits the performance of otherwise unfeasible operations, discloses the true extent of tumors, increases the indications for hepatectomy, and renders surgery safer. Despite this, it has remained relatively neglected in the literature over the past two decades, during which time much progress has been achieved. This is the first atlas on the subject, and it is comprehensive in scope. The state of the art in the use of ultrasound for resection guidance is carefully documented, and new techniques for exploration of the biliary tract and facilitation of transplant surgery are presented. Further important topics include ...

Imaging Tumor Response to Therapy

Imaging Tumor Response to Therapy

Measurement of solid tumor response to treatment relies mainly on imaging. WHO tumor response criteria and, more recently, RECIST (response evaluation criteria in solid tumors) have provided means to objectively measure tumor response in clinical trials with imaging. These guidelines have been rapidly adopted in clinical practice to monitor patient treatment and for therapy planning. However, relying only on anatomical information is not always sufficient when evaluating new drugs that will reduce a tumor's functionality while preserving its size. Finding more reliable and reproducible measures of tumor response is one of the most important and difficult challenges facing modern radiology as it requires an entirely new approach to imaging. The aim of this book is to address the assessment of response to treatment by adopting a multidisciplinary perspective, just as occurs in real life in a comprehensive cancer center. Oncologists and imaging experts consider two cancer models, locally advanced disease and metastatic disease, jointly exploring both conventional and advanced means of measuring response to standard treatment protocols and new targeted therapies.

Biliary Lithiasis

The first laparoscopic cholecystectomy, performed by Prof. Philippe Mouret in 1987 and described by himself in the first chapter of this book, was an event that revolutionized surgery in the past few decades. Although the majority of surgeons today are unfamiliar with the his- ry of early minimally invasive surgery developments, it is important to realize that the advent of laparoscopy led not only to new surgical te- niques, but also to a change in the doctrine of medical care, by streng- ening the concept of minimal invasiveness. This is particularly the case for biliary lithiasis, for which laparoscopy has provided major benefits in terms of both diagnosis and surgical tre- ment. However,...
