Se llama "alto riesgo" al embarazo en el cuál existe, asociado a patología materna y/o fetal, un pronóstico de morbilidad mayor que en un embarazo normal. Estos embarazos representan del 20% al 30% del total, pero son responsables de más del 80% de los resultados maternos y perinatales adversos. Problemas como la prematurez, la asfixia perinatal, las malformaciones congénitas y las enfermedades maternas pregestacionales requieren de estrategias orientadas a su prevención diagnóstico y tratamiento oportunos.
Barría señala que la medicina no debería ser monopolio de los médicos, sino preocupación de todos. Tarde o temprano, señala, todos seremos pacientes. Al nacer nos recibe un médico, atiende nuestra salud durante nuestra vida y nos cuida y acompaña en la muerte. Por ende, no hay un tema más universal y transversal que la medicina. Barría propone una interesante tesis en el sentido de que la participación de los pacientes en el combate de sus patologías tiene en sí un efecto terapéutico. Por ello, propicia un modelo de democracia terapéutica que significa que los pacientes sean protagonistas de una medicina integral que considere su realidad biológica, su psicoemocionalidad y su entorno de afectos y desafectos. Resalta la magistral definición de enfermo de Unamuno: “Un ser humano, de hueso y carne, que sufre, piensa, ama y sueña”. Sin ambages, Barría plantea que, así como la dictadura es tóxica, la democracia es terapéutica tanto en política como en medicina. Aparte de su utilidad práctica para médicos y enfermos, este libro es un baño de optimismo.
