NHS Next Stage Review

This report from the Health Committee examines Lord Darzi's final report of the NHS Next Stage Review (NSR), "High quality care for all" (2008, Cm. 7432, ISBN 9780101743228). The Committee warns that primary care trusts (PCTs) are not yet capable of carrying out the reforms, and highlights insufficient analytical and planning skills and variable management quality within PCTs. Poor PCT commissioning and the failure of successive reforms to enhance it mean that implementation of the reforms may be slower and more uneven than expected. The Committee calls on the Government to publish and rigorously monitor milestones for each stage of implementation and says greater clarification is required o...

Medical Specialisation at the London Hospital

The development of specialty skills in medical and surgical practice in the late 19th and in the 20th century transformed medical practice. For the first time, a patient could visit a doctor with the expectation of an accurate diagnosis and effective treatment. Disease prevention became a realistic proposition. Traditional practice methods became obsolescent, but a yearning for generalist medical wisdom and respect continued until the 2nd World War disrupted progress. In the 19th century, the London Hospital was remarkably open to new ideas, and the Chairman of the Board, Viscount Knutsford, was a master fundraiser. Investment in novel facilities and staff, including the establishment of special departments, and consequent changes in clinical practice led to a growing national and international reputation in clinical practice and education. Specialty skills defined innovations, both in hospital and family practice. More recently, merging St Bartholomew’s, the Royal London Hospital and other hospitals has reactivated the advance of specialism.

This practical manual, written by well-known experts, reviews current indications for the use of IgG concentrates and some other modern immunomodulators and provides fundamental information on present-day immunomodulation in patients (and mice). The book opens by tracing the transition from IgG substitution to IgG immunomodulation and providing expert updates on immunomodulatory indications in autoimmune and inflammatory disorders, including hematologic, neurologic, dermatologic, and other diseases. Basic aspects of IgG concentrates, including methods of production, safety, currently available products, and mechanisms of action, are then discussed. An entire chapter is devoted to the different aspects of immunomodulatory IgG treatment in the bleeding disorder immune thrombocytopenia (ITP). Finally, the transition from polyclonal to monoclonal antibody (mAb) treatment is addressed in detail, covering mAb development, methods, mechanisms of action, adverse effects, and more. Particular attention is paid to the example of anti-CD20 (B-cell) antibody.
