New Concepts in Inflammatory Bowel Disease

Inflammatory bowel disease is a chronic immune-mediated inflammation of the gastrointestinal tract of unknown origin, which includes Crohn's disease, ulcerative colitis, and inflammatory bowel disease of unclassified type. It is associated with different intestinal and extraintestinal manifestations like different neurological and psychiatric disorders. Histology is an important tool in the diagnosis and prognosis of inflammatory bowel disease and has an increasing part in patients' management. The objective of treatment is to make and keep long-lasting remission by immunosuppressive treatment like corticosteroids, thiopurines, and monoclonal antibodies directed against tumor necrosis factor alpha. Therapeutic drug monitoring of thiopurines by measuring levels of their metabolites has been proposed as a potentially effective tool in optimizing therapy in inflammatory bowel disease. Diets and their components influence microbiota of the intestine, function of the epithelial barrier, immune response, and other factors that have an important role in development and treatment of inflammation in the gut mucosa.

Current understanding of physiological characteristics of different populations and responses to environmental stress and exercise is primarily derived from research using male participants. Therefore, the physiological responses to exercise testing, prescription, and training in females should be further characterized and explored, as does knowledge on female-specific health and recovery from exercise. Many things have been said and written on the skeletal and non-skeletal effects of vitamin D, but the largest recent interventional studies are generally negative. How should we thus position ourselves in 2020? Should we say “stop” or “not yet”? Indeed, the aging of the population, as well as new challenges and discoveries, is still triggering interest in this old molecule. In this Special Edition of Nutrients, we invited top experts to give their opinion on this important debate. We also encouraged scientists to submit their latest research on nutritional requirements in the general population and in high-risk groups, as well as treatment strategies, epidemiology, analytical updates and new devices for vitamin D measurement and the effect of vitamin D on bone and extra-skeletal health. As readers will see, this Special Issue reinforces the high prevalence of vitamin deficiency and insufficiency in the general population and supports the safety of this low-cost molecule, revealing new perspectives regarding the extra-skeletal effects of vitamin D.

WHO antenatal care recommendations for a positive pregnancy experience. Nutritional interventions update

The World Health Organization's comprehensive antenatal care (ANC) guideline WHO recommendations on antenatal care for a positive pregnancy experience was first published in 2016 with the objective of improving the quality of routine health care that all women and adolescent girls receive during pregnancy. The overarching principle - to provide pregnant service users with a positive pregnancy experience - aims to encourage countries to expand their health-care agendas beyond survival, with a view to maximizing health, human rights and the potential of their populations. This guideline presents that evidence and updated recommendation on antenatal vitamin D supplements, which updates and does not alter the corresponding recommendation previously issued.
