Few recent events in British Columbia have seized the public mind like the 2006 sinking of the BC Ferries passenger vessel Queen of the North. Across Canada, it was one of the top news stories of the year. In BC it has attained the status of nautical legend. Ten years later, questions are still being asked. How did a ship that sailed the same course thousands of times fall victim to such an inexplicable error? Was the bridge crew fooling around? Why doesn't anybody in the know come forward and tell the truth? Nobody knew the ship, the crew and the circumstances that fateful March night better than the Queen of the North's long-serving captain, Colin Henthorne, and in this book he finally tel...
This book captures current trends and developments in the field of systems thinking and soft operations research which can be applied to solve today's problems of dynamic complexity and interdependency. Such ‘wicked problems’ and messes are seemingly intractable problems characterized as value-laden, ambiguous, and unstable, that resist being tamed by classical problem solving. Actions and interventions associated with this complex problem space can have highly unpredictable and unintended consequences. Examples of such complex problems include health care reform, global climate change, transnational serious and organized crime, terrorism, homeland security, human security, disaster management, and humanitarian aid. Moving towards the development of solutions to these complex problem spaces depends on the lens we use to examine them and how we frame the problem. It will be shown that systems thinking and soft operations research has had great success in contributing to the management of complexity.
Presented in this book are the biographies of Alberta's judiciaries. It serves as a reference for anyone wishing to find information on Alberta's judicial leadership over the past century. By having a compendium of biographies, interested researchers will be able to create interconnections and have accurate historical facts to base their research upon.
This book aims to uncover the root causes of natural and man-made disasters by going beyond the typical reports and case studies conducted post-disaster. It opens the black box of disasters by presenting ‘forensic analysis approaches’ to disasters, thereby revealing the complex causality that characterizes them and explaining how and why hazards do, or do not, become disasters. This yields ‘systemic’ strategies for managing disasters. Recently the global threat landscape has seen the emergence of high impact, low probability events. Events like Hurricane Katrina, the Great Japan Earthquake and tsunami, Hurricane Sandy, Super Typhoon Haiyan, global terrorist activities have become the...

Analysis of the causes of poor flight discipline, case studies of the consequences, and a plan for individual improvement Flight Discipline is the complete tool kit for any aviator, whether military, commercial, or recreational, to develop the crack discipline needed to be a safe and effective aviator. Major Tony Kern analyses the causes of poor flight discipline, gives chilling case studies of the consequences, and lays out a plan for individual improvement. Are always good people doing good things and bad people doing bad things? What is organizational blindness, and how do you protect it? In this book, you will learn how good people do bad things even without recognizing it. Pressures in the aviation environment are explained. Real-life case studies are discussed, and the reader of the book is expected to have certain knowledge about the forces in organizations and a basic understanding of the aviation domain. Aviation SMS (Safety Management Systems) is the formal, top-down, organization-wide approach to managing safety risk and ensuring the effectiveness of safety risk controls. An aviation safety manager is required to understand these forces, and organizations are expected to realize their own blindness and manage these associated risks.

Judgment on Erebus is narrative nonfiction at its most compelling and unsettling. Commanded by one of Air New Zealand’s most meticulous and cautious pilots, a sightseeing airliner inexplicably crashes into an active Antarctic volcano in broad daylight, causing the world’s fourth-worst aviation disaster. The New Zealand government’s Office of Air Accidents Investigation soon publishes an official report attributing the disaster to pilot error. Skeptical, an aroused public demands an “independent” official inquiry. Realizing that he badly needs a second investigator to confirm the first one’s findings, an imperious Prime Minister selects for the post a distinguished High Court judg...
This book provides the first comprehensive comparison of the Aircraft Maintenance Program (AMP) requirements of the two most widely known aviation regulators: the European Aviation Safety Agency (EASA) and the Federal Aviation Administration (FAA). It offers an in-depth examination of the elements of an AMP, explaining the aircraft accident investigations and events that have originated and modelled the current rules. By introducing the Triangle of Airworthiness model (Reliability, Quality and Safety), the book enables easier understanding of the processes by which an aircraft and its components are deemed to be in a safe condition for operation from a cost-effective and optimization perspective. The book compares the best practices used by top airlines and compiles a series of tools and techniques to improve the standards of the AMP. Aircraft maintenance engineers, students in the field of aerospace engineering, and airlines staff, as well as researchers more widely interested in safety, quality, and reliability will benefit from reading this book
