This textbook reviews the novel techniques employed in corneal transplantation. It will assist fellows and corneal surgeons in using these techniques to best effect and in selecting patients for surgical procedures, taking into account the benefits and risks. Until 15 years ago the state-of-the art type of corneal transplantation was penetrating keratoplasty. Since the start of this millennium, however, important advances have been made in developing new surgical techniques. Today, the vast majority of keratoplasty procedures are performed as delicate lamellar procedures, either with the assistance of fine microkeratomes or femtosecond lasers or using very advanced surgical dissection procedures. "Essentials in Ophthalmology" is a new review series covering all of ophthalmology categorized in eight subspecialties. It will be published quarterly, thus each subspecialty will be reviewed biannually. Given the multiplicity of medical publications already available, why is a new series needed? Consider that the half-life of medical knowledge is estimated to be around 5 years. Moreover, it can be as long as 8 years between the first description of a medical innovation in a peer-reviewed scientific journal and publication in a medical textbook. A series that narrows this time span between journal and textbook would provide a more rapid and efficient transfer of medical knowledge into clinical practice, and enhance care of our patients.

Abstract: Background and aim The continuous recording of eye injuries caused by fireworks around New Year's Eve over 6 years allows us to investigate injury numbers, injury patterns, and accident histories in a year to year comparison. Adequate HLA Matching in Keratoplasty

Immunologic privilege of the cornea and anterior chamber has, in most cases, been believed to be a sufficient guarantee for survival of the donor cornea. If this failed, it was thought that adjunctive immunomodulating agents would compensate. But the question of how to care for better tolerance and safer long-term survival of corneal grafts after withdrawal of a potent immunomodulating shield still remained unanswered. The answer is human leukocyte antigens (HLA) matching. In this volume, immunological and clinical aspects are discussed by renowned clinical and experimental immunologists as well as experienced keratoplasty surgeons. These experts conclude that HLA matching is, in fact, a valuable adjunct for long-term survival of corneal grafts not only in those at high risk but notably also in patients at normal risk. This volume opens new pathways towards the successful application of HLA matching to keratoplasty surgeons, experimental and clinical immunologists and cornea bank officers, at the same time acting as an aid to ophthalmologists when finding the basis to adequately advise their keratoplasty patients.
