Exciting major changes have occurred in the understanding and treatment of gallstone disease over the last two decades. In bygone years, books about gallstones were often based on postgraduate lectures which the author, usually a surgeon of distinction, had given. More recently, many books dealing with this subject have been based upon national or international conferences. The single-author text has the disadvantage that few authors today can authoritatively encompass a whole field: the reports of symposia, conferences or workshops often lack balance and auth ority. The contributions to this volume cover all aspects of the assessment and management of hepatobiliary disease. The focal points of the book consist of three state-of-the-art summaries. The first of these deals with the highly topical problem of liver transplants from the point of view of patient selection. The second considers drug-induced liver injury in view of the fact that the liver is the main metabolic site for a number of drugs. The final summary deals with liver and aging: it asks whether the liver follows the aging process of the host organisms and whether the liver of aged liver transplant candidate donors could be suitable for grafting. Aside from these topics, the volume presents basic research on hepatic transport mechanisms, intrahepatic cholestasis and gall-stone disease, which serves as a background for the topics more specifically concerning the assessment of liver function. Much of the book is then devoted to the management of the commonest forms of liver diseases and their complications, such as chronic active hepatitis, liver cirrhosis, portal hypertension, hepatic encephalopathy, hepatorenal syndrome, and ascites.

Twenty years ago the very idea of an international conference on the fiber contained in plant food would have been totally inconceivable. At that time fiber was generally viewed as an inert component of food of no nutritional value and consequently consid ered as a contaminant, the removal of which would enhance the purity of a product. It was measured by a now obsolete and almost worthless test introduced in the last century for veterinary rather than human nutrition, and what was measured was referred to as "crude fiber," containing part of the cellulose and lignin but none of the numerous components of fiber now known to play important roles in the maintenance of health. There were a few lone voices prior to the last two decades who had extolled the laxative properties of the undigested portion of food, assuming that these were related to its irritant action on the bowel mucosa. In retrospect this was a total misconception, and "softage" would have been a more appropriate term than "roughage," since its presence insured soft, not irritating, colon content.
