So beteiligten sich Ärzte als Kliniker, Wissenschaftler und Gutachter an Zwangssterilisationen, Krankenmorden (NS-"Euthanasie") und Menschenversuchen in den Konzentrationslagern. Ärzte wirkten aktiv und fast ausnahmslos freiwillig mit am Holocaust und am Völkermord an Sinti und Roma. Die Heilpädagogische Abteilung als Evaluierungsinstanz kindlichen Verhaltens entfaltete ihre Wirkmacht im Zusammenspiel mit den Behörden, die sie konsultierten. Tausende Minderjährige wurden von 1911 bis 1949 begutachtet und waren als Folge vielfach von einschneidenden Lebensveränderungen betroffen. Eltern, Jugendämter, Schulen, Gerichte und Ärzt*innen wandten sich mit dem Wunsch nach Begutachtung ihrer Schützlinge ab 1911 regelmäßig an die neugegründete Heilpädagogische Abteilung. Aufenthalte von einer Woche bis zu mehreren Monaten sollten durch Beobachtung und Testungen die Einschätzung von Verhaltensweisen und deren bestmögliche Förderung oder Korrektur gewährleisten. Diese schließlich umzusetzen war Aufgabe der Einrichtungen und Personen, die die entlassenen Patient*innen zusammen mit einer Maßnahmenempfehlung in Empfang nahmen. Geprägt von den ärztlichen Leitungsfiguren änderten sich in der 1. Hälfte des 20. Jahrhunderts konzeptuelle Ansätze, jedoch nicht Funktionsweise, Aufgaben und Kooperationen der Abteilung. Sie firmierte als klinisch-wissenschaftlicher Leitstern im Wiener Fürsorgesystem.
