Unhealthy Politics

  • Categories: Law

How partisanship, polarization, and medical authority stand in the way of evidence-based medicine The U.S. medical system is touted as the most advanced in the world, yet many common treatments are not based on sound science. Unhealthy Politics sheds new light on why the government's response to this troubling situation has been so inadequate, and why efforts to improve the evidence base of U.S. medicine continue to cause so much political controversy. This critically important book paints a portrait of a medical industry with vast influence over which procedures and treatments get adopted, and a public burdened by the rising costs of health care yet fearful of going against "doctor's orders." Now with a new preface by the authors, Unhealthy Politics offers vital insights into the limits of science, expertise, and professionalism in American politics.

Speaking for the Dying

Seven in ten Americans over the age of age of sixty who require medical decisions in the final days of their life lack the capacity to make them. For many of us, our biggest, life-and-death decisions—literally—will therefore be made by someone else. They will decide whether we live or die; between long life and quality of life; whether we receive heroic interventions in our final hours; and whether we die in a hospital or at home. They will determine whether our wishes are honored and choose between fidelity to our interests and what is best for themselves or others. Yet despite their critical role, we know remarkably little about how our loved ones decide for us. Speaking for the Dying ...

From 2000-01 to 2005-06, rates of hip, knee, and shoulder replacement all rose substantially among Medicare beneficiaries, and there was widespread variation in the use of these procedures across geographic regions and by race. The Dartmouth Atlas Project has detailed similar variation in the use of other elective procedures. One possible interpretation of this variation is that the decision to undergo the procedure may be influenced more by physician judgments than by the preferences and values of the individual patients. Another explanation might be that patients in some geographical areas do not have adequate access to joint replacement. These findings highlight the need for improved physician and patient education and the use of shared decision-making to determine whether a patient should undergo joint replacement.

This major new work updates and significantly expands The Hastings Center's 1987 Guidelines on the Termination of Life-Sustaining Treatment and Care of the Dying. Like its predecessor, this second edition will shape the ethical and legal framework for decision-making on treatment and end-of-life care in the United States. This groundbreaking work incorporates 25 years of research and innovation in clinical care, law, and policy. It is written for physicians, nurses, and other health care professionals and is structured for easy reference in difficult clinical situations. It supports the work of clinical ethicists, ethics committee members, health lawyers, clinical educators, scholars, and policymakers. It includes extensive practical recommendations. Health care reform places a new set of challenges on decision-making and care near the end of life. The Hastings Center Guidelines are an essential resource.

Leaving the hospital sounds simple. But all too often, patients find themselves back in a hospital bed--or even the emergency room--within a matter of weeks of going home. This article will help you LOOK at the care you get and understand what good care for patients who are leaving the hospital looks like, help you LEARN what you can do to make sure you get the best possible care, and help you LIVE better by taking action to get better care.

Our Changing Journey to the End

This novel, cross-disciplinary collection explains how dying, death, and grieving have changed in America, for better or worse, since the turn of the millennium. What does dying with dignity mean in a diverse society with rapidly advancing technology, an aging population, and finite resources? In this fascinating collection, scholars from across the nation illuminate the remarkable changes that have taken place in recent years, are now underway, and loom on the horizon as they lead readers on an exploration of the ways Americans think about and handle dying and death. Volume 1, New Paths of Engagement, addresses changes in the circumstances and expressions of death, dying, and grief in 21st-century America. Volume 2, New Venues in the Search for Dignity and Grace, delves into the challenges inherent in creating a medical and social system that allows for an optimal end-of-life experience for all and proposes ways in which society can be reshaped to move toward that ideal.
