In the nineteenth century the distinction between the specialist and the general practitioner was mainly one of social class - ori- ginating in the distinction between the apothecary and the physi- cian or surgeon. In the inter-war period there were signs that the distinction was breaking down. But rigidity in the present distincti- on between hospital and general practice was introduced by the nati- onal health service in 1948. What was the process by which the medi- cal profession decided to turn the ancient class division of the pro- fession into a distinction between hospital-based specialist practice and community-based general practice? Why did consumers and their political representatives accept this? These are the questions the author seeks to answer from a meticulous study of the vast document- ary evidence which he has assembled.
