Background - AimPost-extubation laryngeal spasm is a rather common complication following ENT procedures. La spermatogenèse est le processus de production de cellules germinales haploïdes. Le but de cette thèse a été d'étudier l'implication potentielle d'une nouvelle classe des petits ARNs, les microARNs, dans le développement testiculaire et la spermatogénèse. Nous avons montré que l'invalidation génique du "Dicer (Dcr)", une endonuclease RNase III essentielle pour la production des microARNs matures, spécifiquement dans les cellules Sertoli (SCs), induit une infertilité complète causée par l'incapacité des SCs à maturer et soutenir la spermatogénèse. En absence de SC-Dcr, nous avons détecté des altérations significatives du transcriptome et du protéome testiculaire. Ceci nous a permis d'identifier de nouveaux gènes qui constituent des cibles directes ou indirectes des microARNs Sertoliens, et dont la dérégulation pourrait expliquer la dysgénésie testiculaire et l'azoospermie. L'ensemble de nos résultats souligne l'importance du contrôle post-transcriptionel lors de la spermatogenèse, et ouvre ainsi des nouvelles perspectives pour l'étude de la fonction testiculaire.

Aim of present study was the comparison between transdermal fentanyl patches vs subcutaneous continuous infusion elastomeric pumps in end stage cancer patients, pain management.Material and MethodsIn a randomized perspective study, 44 end stage cancer patients suffering from severe pain (VAS score >6) were assigned in two groups: (Group A, n=22 and group B=22). In group A patients transdermal fentanyl patches (dose according to age, weight, general condition, liver function and pain intensity) were prescribed, while in group B patients a subcutaneous. continuous, dose modified, elastomeric pump of 270 ml containing tramadol 100-300mg/24hr, parecoxib 40-80mg/24hr (depending on patientsu2019 renal function, clonidine 150mcg/24hr, ranitidine 100mg/24hr,and dexamethasone 8mg/24hr. Pain scores (according to VAS scale), patientsu2019need for rescue analgesics (sublingual fentanyl tabets), side effects, and patientsu2019 collaboration were recorded.Results Patientsu2019 study in a three month period showed statistically signignificant (p
