Abstract: Background Although antihyperglycemic pharmacotherapy in frail older adults with type 2 diabetes mellitus (T2DM) is challenging, recommendations from international guidelines are mainly based on indirect evidence from trials not including frail participants. Objective This systematic review investigated the effectiveness and safety of pharmacotherapy in frail older adults with T2DM. Material and methods Randomized (RCT) and non-randomized prospective clinical trials (non-RCT) were searched in three electronic databases (Medline, Embase, Central) up to October 2018. Trials in older adults with T2DM who were assessed as significantly or severely impaired by defined cut-off scores of ...

Abstract: Background The association of frailty based on the accumulation of deficits with postoperative delirium (POD) has been poorly examined. We aimed to analyze this association in older patients undergoing elective surgery. Methods Preoperative data was used to build a 30-item frailty index (FI) for participants of the PAWEL-study. Delirium was defined by a combination of I-CAM and chart review. Using logistic regressions models we analysed the association between frailty and POD adjusting for age, sex, smoking, alcohol consumption, education and type of surgery. Results Among 701 participants (mean age 77.1, 52.4% male) median FI was 0.27 (Q1 0.20| Q3 0.34), with 528 (75.3%) frail par...

Aging between Participation and Simulation

With increasing urgency, decisions about the digitalized future of healthcare and implementations of new assistive technologies are becoming focal points of societal and scientific debates and addresses large audiences. Decisions require a careful weighing of risks and benefits and contextualizing in-depth ethical analysis with robust empirical data. However, up to now, research on social assistive technologies is mostly dispersed over different academic fields and disciplines. A comprehensive overview on discussions regarding values at stake and ethical assessment of recent developments especially in healthcare is largely missing. This publication initiates an interdisciplinary discourse on...

Praxishandbuch Altersmedizin

For reasons of demographic changes alone, expertise in geriatric medicine is becoming increasingly important in all areas of clinical care. Appropriate treatment care for the elderly requires complex work at the intersection between geriatrics, gerontopsychiatry and interventional gerontology. This interdisciplinary standard work provides the reader with a systematic, wide-ranging and up-to-date summary of knowledge from all three disciplines that is relevant for diagnosis, differential diagnosis, practical therapy implementation and care planning. Against the background of essential basic concepts that support practical activity (Part I), the main part (Part II) presents the practical clini...

Current understandings of ageing and diversity are impoverished in three main ways. Firstly, with regards to thinking about what inequalities operate in later life there has been an excessive preoccupation with economic resources. On the other hand, less attention has been paid to cultural norms and values, other resources, wider social processes, political participation and community engagement. Secondly, in terms of thinking about the ‘who’ of inequality, this has so far been limited to a very narrow range of minority populations. Finally, when considering the ‘how’ of inequality, social gerontology’s theoretical analyses remain under-developed. The overall effect of these issues...

Lifestyle-integrated Functional Exercise (LiFE) program to prevent falls

The Lifestyle-integrated Functional Exercise (LiFE) program is a way of reducing the risk of falls by integrating balance and strength activities into regular daily tasks. Unloading the dishwasher becomes an opportunity to improve strength. Brushing your teeth becomes an opportunity to improve balance. In the LiFE program, every daily task becomes an opportunity to improve balance and strength. This is a different approach to a traditional program where you would be required to complete a series of exercises a certain number of times a day for a set number of days each week. The trainer's manual outlines the principles of the LiFE program and provides a step-by-step guide for therapists and trainers to implement the program with their clients. It should be used in conjunction with the participant's manual so that the program is fully understood from both the trainer's and participant's perspectives.
