Abstract: Prognostic assessment of patients with liver cirrhosis allocated for implantation of a transjugular intrahepatic portosystemic shunt (TIPS) is a challenging task in clinical practice. The aim of our study was to assess the prognostic value of the CLIF-C AD (Acute Decompensation) score in patients with TIPS implantation. Transplant-free survival (TFS) and 3-month mortality were reviewed in 880 patients who received de novo TIPS implantation for the treatment of cirrhotic portal hypertension. The prognostic value of the CLIF-C AD score was compared with the Model for End-Stage Liver Disease (MELD) score, Child-Pugh score, and albumin-bilirubin (ALBI) score using Harrell's C concordance index. The median TFS after TIPS implantation was 40.0 (34.6-45.4) months. The CLIF-C AD score (c = 0.635 [0.609-0.661]) was superior in the prediction of TFS in comparison to MELD score (c = 0.597 [0.570-0.623], P = 0.006), Child-Pugh score (c = 0.579 [0.552-0.606], P 0.001), and ALBI score (c = 0.573 [0.545-0.600], P

Abstract: Statins induce plaque regression characterized by reduced macrophage content in humans, but the underlying mechanisms remain speculative. Studying the translational APOE*3-Leiden.CETP mouse model with a humanized lipoprotein metabolism, we find that systemic cholesterol lowering by oral atorvastatin or dietary restriction inhibits monocyte infiltration, and reverses macrophage accumulation in atherosclerotic plaques. Contrary to current believes, none of (1) reduced monocyte influx (studied by cell fate mapping in thorax-shielded irradiation bone marrow chimeras), (2) enhanced macrophage egress (studied by fluorescent bead labeling and transfer), or (3) atorvastatin accumulation in...

Proton Pump Inhibitor Treatment Aggravates Bacterial Translocation in Patients with Advanced Cirrhosis and Portal Hypertension

Abstract: Recent studies have linked proton pump inhibitor (PPI) treatment to increased complications of cirrhosis, such as bacterial infections and hepatic encephalopathy. However, the underlying pathophysiological mechanisms remain unclear. The present study investigated the hypothesis that PPI treatment may promote adverse effects in patients with advanced cirrhosis by affecting subclinical bacterial translocation (BT) from the gut into the portal venous bloodstream. Blood samples from the portal vein were obtained during implantation of a transjugular intrahepatic portosystemic shunt (TIPS) in a total of 80 cirrhosis patients with PPI treatment (PPI group, n = 57) and without PPI treatme...

Drawing Futures brings together international designers and artists for speculations in contemporary drawing for art and architecture.Despite numerous developments in technological manufacture and computational design that provide new grounds for designers, the act of drawing still plays a central role as a vehicle for speculation. There is a rich and long history of drawing tied to innovations in technology as well as to revolutions in our philosophical understanding of the world. In reflection of a society now underpinned by computational networks and interfaces allowing hitherto unprecedented views of the world, the changing status of the drawing and its representation as a political act ...

Glacial Geology

The new Second Edition of Glacial Geology provides a modern, comprehensive summary of glacial geology and geomorphology. It is has been thoroughly revised and updated from the original First Edition. This book will appeal to all students interested in the landforms and sediments that make up glacial landscapes. The aim of the book is to outline glacial landforms and sediments and to provide the reader with the tools required to interpret glacial landscapes. It describes how glaciers work and how the processes of glacial erosion and deposition which operate within them are recorded in the glacial landscape. The Second Edition is presented in the same clear and concise format as the First Edit...

This book, the first complete textbook on this novel field in Medicine, comprehensively covers the clinical presentation, pathogenesis, genetics, and latest management strategies for autoinflammatory disorders as well as the basic science of autoinflammation. Relevant concepts such as how translational science of genetics and immunology relates to the innate immune system and autoinflammation are covered. Descriptions of the monogenic and polygenic/complex diseases that fall under the umbrella of autoinflammatory diseases are provided. Further topics covered include the latest clinical and genetic diagnostic approaches, concepts on the relationship between autoinflammation and autoimmunity/immunodeficiency, the role of autoinflammation in cancer, treatments and management strategies for these diseases, and potential areas of future development. The Textbook of Autoinflammation systematically describes and reviews diagnostic and treatment options for autoinflammatory disorders as well as all aspects of the concept of autoinflammation, and represents a valuable resource for professionals in a variety of disciplines who encounter these patients or who study autoinflammation.

The Monocle Book of the Nordics

The Monocle team heads north in this celebration of all things Nordic. This heavily illustrated book from Monocle is a celebration of the Nordic region, featuring some surprises, quirks—and maybe a sauna or two— along the way. Monocle’s journalists, editors, and photographers have returned time and again to all corners of northern Europe for insights, inspiration, and ideas for living better. This book isn’t about hammering the overhyped hygge trend or fussing over foamy food. Much the opposite—it’s about a shared but distinct set of values that have helped these nations excel in quiet diplomacy, thoughtful design, and reasoned debate. Monocle looks beyond the clichés and uncove...

Land, Labor and the Origins of the Israeli-Palestinian Conflict, 1882-1914

Gershon Shafir challenges the heroic myths about the foundation of the State of Israel by investigating the struggle to control land and labor during the early Zionist enterprise. He argues that it was not the imported Zionist ideas that were responsible for the character of the Israeli state, but the particular conditions of the local conflict between the European "settlers" and the Palestinian Arab population.
